Gateway to Care (non-profit collaboration of 165+ member and affiliated organizations)

  • Navigation, Outreach & Educaton                                                                                                                                           Navigation services provide assistance to under-served populations seeking to obtain needed health access and social services assistance. Through the Navigation, Outreach and Education programs, State Certified Community Health Workers provide education, information and one-on-one assistance on health and community resources. They offer training and direct application assistance in enrollment for CHIP/Medicaid, and other federal and regional indigent financial assistance programs, with a focus on uninsured and under-insured populations. Additionally, Navigators educate clients on the importance of locating and using a health home to achieve and maintain good health and link them to other community resources.
